As a participant of the event entitled 200th Anniversary of the Founding of Wiener Städtische Versicherungsverein organised by KOOPERATIVA poisťovňa, a.s. Vienna Insurance Group, with registered office at Štefanovičova 4, 816 23 Bratislava, ID No.: 00 585 441, and KOMUNÁLNA poisťovňa, a.s. Vienna Insurance Group, with registered office at Štefánikova 17, 811 05 Bratislava, ID No.: 31 595 545, which will take place on 21 November 2024 at the Slovak Philharmonic in Bratislava, I hereby by my signature grant my consent to the Insurance Company for the processing of my personal data, namely: in the scope of first name, last name, e-mail and my likeness in the form of an image record (photograph) made at any time during the above-mentioned event for the purpose of publishing the above-mentioned personal data within the internal communication tools of the Insurance Company (e.g. the internal magazine Impulz, intranet and others), external communication on LinkedIn and archiving related to these internal communication tools.
I acknowledge that my consent to the processing of my personal data may be revoked at any time in writing to the address of the Insurance Company or by e-mail to vargovaka@koop.sk.
I declare that I have been provided with information about the processing of my personal data pursuant to Article 13 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46/EC (General Data Protection Regulation) (hereinafter also referred to as the "Regulation"). Detailed information on the processing of personal data, including my rights as a data subject, can be found on the Insurance Company's intranet at https://kooperativa.sk/ochrana-osobnych-udajov. I also declare that I have been informed that the processing of the personal data provided by me may also be entrusted by the controller to the processor IM Print SK s.r.o., with registered office at Družstevná 30, 900 66 Vysoká pri Morave and Design friendly, s.r.o., with registered office at Sládkovičova 32, Modra 900 01, ID No. 44 608 781, to the extent necessary for the graphic work and printing of the respective issues of the Impulz magazine.
I also declare that the personal data provided have been freely given.
